Users bypassing Product Rules

I have a couple of product rules setup that ensure that 1 Base Product exists on every subscription that we quote.  When I test my rule which just checks for a rollup of one base product, it all seems to work for myself and my QA team.  We are unable to continue and save the quote without going back and fixing the mandatory Base Product rule.


Somehow after a couple of months a couple of users have somehow gotten a quote through and processed without a Base Product.  


Does anyone know how that might work?  I can't find the series of steps or situation that would allow me through the validations.
